Project 2025 Health Care Proposals
Project 2025 proposes a dramatic rollback of federal health care support, aiming to reduce coverage, cut funding, and reshape public health institutions along conservative ideological lines.
Project 2025, developed by the Heritage Foundation and over
100 conservative organizations, outlines a sweeping vision for restructuring
the federal government, including its role in health care. Its health policy
agenda centers on dismantling key components of the Affordable Care Act (ACA),
slashing Medicaid and other safety-net programs, and curbing the authority of
public health agencies.
One of the most consequential proposals is the rollback of
Medicaid expansion and deep cuts to federal investment in Medicaid, the
Children’s Health Insurance Program (CHIP), and ACA marketplace subsidies. The
Republican Study Committee’s budget, aligned with Project 2025, calls for $4.5
trillion in cuts to these programs Center
on Budget and Policy Priorities. These changes would likely result in
millions losing coverage, especially low-income adults, children, and people
with disabilities. The plan would shift more financial responsibility to
states, which may lack the resources or political will to maintain coverage
levels.
Project 2025 also targets consumer protections established
under the ACA. While not explicitly repealing the ACA, its proposals would
undermine coverage standards, allowing insurers to offer plans with fewer
benefits and higher costs—particularly for people with pre-existing conditions Center
on Budget and Policy Priorities. This could lead to a proliferation of
“skimpy” plans that fail to cover essential services, reversing recent gains in
coverage and affordability.
Beyond insurance, Project 2025 seeks to restructure federal
health agencies. It proposes splitting the Centers for Disease Control and
Prevention (CDC) into two entities—one for data collection and another for
policy guidance—arguing that the CDC should not make policy decisions American
Public Health Association. It also recommends limiting the CDC’s authority
to issue guidance on vaccines and masks, leaving such decisions to parents and
providers. The Food and Drug Administration (FDA) would face reforms to its
drug approval process, and the National Institutes of Health (NIH) would see
leadership term limits and reduced funding, with more money directed to states
instead of federal research institutions American

Reproductive health is another focal point. Project 2025
promotes a strict anti-abortion agenda, including efforts to combat “abortion
tourism,” restrict access to abortion medications like mifepristone, and
mandate detailed abortion reporting by states American
Public Health Association. It also seeks to limit access to contraception
and gender-affirming care, and to halt federal data collection on gender
identity.
In sum, Project 2025 envisions a health care system with
reduced federal oversight, diminished public health infrastructure, and fewer
protections for vulnerable populations. Its proposals reflect a broader
ideological shift toward decentralization, privatization, and moral
conservatism in health policy. If implemented, these changes would likely
increase the number of uninsured Americans, reduce access to comprehensive
care, and weaken the government’s ability to respond to public health crises.
